Incident 02 Aug 1970 A male passenger drew a pistol and a bottle he said claimed nitroglycerin. He demanded to be flown to Cuba. This was the first hijacking of a Boeing 747. W/o 03/27/77 when colided with another KLM 747 (PH-
BUF) on runway in low visibility. 335 of 398 killed plus all 248 on KLM. Worst aviation disaster in history.

DBR on landing in storm 08/04/83 KHI. Veered off runway due to Inop reverser on #4, and hit solid ground fixtures. All 243 ok.

w/o 09/06/70 CAI Flight 93 hijacked by 2 terrorists after takeoff in AMS on the way to JFK Airplane was diverted to BEY where 7 other hijackers boarded and airplane flew to Cairo where all 153 passengers and 7 crewmembers evacuated just before the plane was blown up after landing Airplane had logged only 1125 hours. 3 other aircraft were simultaneously hijacked, a Swissair DC-
8, TWA B707 and a BOAC Super VC10. All were blown up at Dawson's field, Jordan. The PA 747 was originally directed to the same destination but Captain said the field was too short for safe landing.

Crashed on logistic flight from Tehran to McGuire AFB via Madrid 5/9/76 While approaching Madrid in heavy thunderstorm Crew requested radar vector diversion of the storm while on apporach to Barajas when lighting struck airframe and discharged on the left wing static discharger During its path it ignited fuel vapors in left wing fuel tank and explosion ripped skin off the top of the left wing, causing severe oscilation that led to left wing separation 17 souls aboard died

LPA collision with taxiing Pan Am 747 (N736PA). KLM took off runway 30 in fog with Pan Am still taxiing on it. When both airplanes spotted each other KLM rotated dragging its tail while PAN AM applied full power and attempted to turn right off runway. KLM 747 took off, but sheared off the top of PAN AM 747 and crashed 300m down. All 248 souls aboard KLM together with 335 PAN AM souls perished making this the worst accident to date. KLM logged 21195 hours on 5202 cycles. Both airplanes were diverted to TCI because of a bomb threat in LPA.

destroyed 5/7/90 during landing operating flight AI132 from London Heathrow to Delhi. After reverse thrust was deployed the No.1 engine pylon failed due to the improper installation of a diagonal-
brace aft fuse- pin which reduced the load carrying ability of the pylon and excessive force accentuated by the heavy landing lead to structural failure causing the engine to tilt forward. Heat from the engine set the left wing on fire. None of 215 souls aboard (195 pax 20 crew) were hurt.

OSA Airplanes aft pressure bulkhead rupture resulting in loss of hydraulics and most of rudder. After 30 minutes of uncontrollable flight the airplane brushed against tree tops of one ridge and crashed into the next 100km northwest of Tokyo. Airplane had been damaged in the past when the tail struck the runway on landing. Repairs by Boeing to the bulkhead in 1978 were incorrect (one row of rivets instead of two) contributed to its failure when JA123 reached cruising altitude 520(524). TT 25030 TC 18835

Cause determined as probable inflight breakup following incorrect repair and fatigue of rear bulkhead after tailstrike in 1980, similar to JAL JA8119 accident in 1985.
